Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team in Ann Arbor, MI. The RN III role involves advanced training and leadership responsibilities, helping patients navigate care, coordinate services, and support transitions of care.

The position is part-time with evening shifts, offering opportunities to lead clinical staff, educate patients, and collaborate with a multidisciplinary team to improve outcomes and the patient experience.

Qualifications

• BSN strongly preferred or graduate of an accredited Registered Nurse program.
• Advanced certifications relevant to specialty (e.g., AAACN, IBCLC) or graduate degree preferred.
• RN license in Michigan (unrestricted) and CPR certification.

Responsibilities

• Nurse leader who assists management with hiring, motivating and engaging clinical staff.
• Identify and target at-risk patients to improve health outcomes.
• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support transitions of care and post-discharge follow-up.
