Job summary

Trinity Health IHA Medical Group in Michigan is seeking a compassionate Registered Nurse to join our outpatient care team. In this role, you’ll perform clinical triage, care coordination, patient education, and ongoing support to improve outcomes and enhance the patient experience.

You will assess patient needs by phone and in-office visits, collaborate with providers and care managers, manage medications and referrals, and help patients navigate barriers to care.

Qualifications

• Graduate of an accredited RN program.
• Valid, unrestricted Michigan RN license with CPR certification.
• 1–2 years of clinical experience in a medical or physician office.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
