Qualifications

• RN license in Michigan (unrestricted). CPR certification required.
• BSN preferred or accredited RN program graduate.
• 3 years of clinical experience in ambulatory care or RN II level competency.

Responsibilities

• Triage and assess patient needs by phone and in-office visits.
• Provide clinical guidance, education, and care coordination.
• Support transitions of care and post-discharge follow-up.
• Identify at-risk patients and coordinate extensive care.
• Lead and assist management with staff hiring and engagement.
• Manage referrals, test results, and prior authorizations.
