Children's Health in Dallas, TX is seeking a Clinical Pharmacist for our Outpatient Pharmacy. The role is on-site with PRN/as-needed scheduling and ideal evening availability to support patient care needs.

You will collaborate with a multidisciplinary team to optimize medication use, provide pharmacotherapy consultations, and contribute to quality improvement initiatives within the outpatient setting.

Competenze

• Doctorate of Pharmacy completed with residency in pediatrics.
• Licensed pharmacist with Texas license eligibility.
• Interest in pediatric outpatient pharmacotherapy.

Mansioni

• Provide pharmaceutical services in assigned outpatient areas.
• Review orders for appropriateness and perform final verification.
• Advise on medication therapy optimization and pharmacotherapy consultations.
• Deliver drug information to physicians, nurses, and families.
• Monitor for drug interactions, duplication, and safety concerns.
• Participate in transitions of care and patient access issues.
• Support Epic order entry and practice area workflow improvements.
• Educate staff, residents, and students; precept where applicable.
