Job summary

Anchor Health is seeking an After-Hours Hospice LVN/RN to provide skilled, on-call nursing care during evenings, nights, and weekends. The position requires strong clinical judgment and excellent communication skills to ensure continuity of care and emotional support for patients and families.

Qualifications

• Current RN or LVN license in good standing.
• Hospice or home health experience preferred.
• Strong clinical judgment and ability to work independently.
• Excellent communication and problem-solving skills.
• Compassionate presence in emotionally sensitive situations.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Respond to after-hours patient calls and urgent care needs.
• Perform skilled nursing assessments and symptom management.
• Provide emotional support and education to patients and families.
• Implement and update patient plans of care as needed.
• Communicate patient condition changes to the interdisciplinary team.
• Collaborate with physicians and caregivers regarding patient needs.
• Complete accurate and timely clinical documentation.
• Uphold hospice philosophy, policies, and regulatory standards.
