Job summary

Heartland Hospice in Missouri is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays.

You will work as the primary liaison among patients, families, physicians, and the hospice team, documenting visits and managing symptoms.

This role emphasizes independence, compassionate care, and collaboration with interdisciplinary staff to maintain continuity of care.

Qualifications

• Current RN license in the state of employment or eligibility to obtain license.

Responsibilities

• Serve as the primary liaison between patients, families, physicians, and the hospice team during shifts.
• Respond promptly to urgent calls, patient visits, and symptom management needs after hours.
• Document clinical visits and communications accurately and timely.
• Collaborate with LPNs/LVNs, CNAs, and other support staff to coordinate care.
• Maintain and implement the Plan of Care as prescribed by the physician.
• Participate in quality assessment and performance improvement programs.
