Job summary

Redwood Grove Post Acute in Santa Cruz, CA is seeking a licensed LVN to provide compassionate care on the PM shift (3:00pm–11:30pm). You will record histories, monitor vitals, and support patients daily while collaborating with a multidisciplinary team.

Active California LVN license required. Prior experience and strong communication are essential to support residents and families with informed, empathetic care.

Qualifications

Proven experience as LVN or similar role.
Undertake diverse care tasks within a multidisciplinary team.
Familiarity with health, safety and sanitation standards.
Understanding of confidentiality obligations and nursing best practices.
Strong knowledge of nursing terminology and documentation.
Competent data entry and basic computer tasks using MS Office.

Responsibilities

Record medical history accurately.
Take and record vital signs (blood pressure, temperature, etc.).
Monitor patient condition and progress; document charts.
Assist with activities of daily living and patient comfort.
Administer injections and prescribed medications; wound care as needed.
Collect samples for lab testing as assigned.
Provide emotional support and communicate with families.
