Job summary

Civita Care at Bayview in Waterford, CT is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care under the supervision of the RN. This role involves supervising nursing assistants to ensure high-quality care for residents in a respectful, patient-centered environment.

New graduates are welcome. Applicants must have an approved LPN/LVN program credential, CPR certification, and an active CT license.

Qualifications

Must be a graduate of an approved LPN/LVN program.
Must be CPR certified.
Must possess a current, unencumbered license to practice as an LPN/LVN in Connecticut.

Responsibilities

Meet the nursing and rehabilitation needs of each resident.
Maintain current resident care plans on all residents under care.
Follow physician’s orders and render/assist with nursing care, treatments, and dressings.
Transcribe physician’s orders promptly and accurately.
Review patient records daily for accuracy and completeness.
Document comprehensive nursing notes and visit residents daily to assess needs.
Maintain awareness of residents on alert list and communicate findings to physicians.
Administer medication according to procedure and control drugs safely.
Initiate emergency procedures as necessary and uphold safety techniques.
Contribute to effective communication and positive resident-family relations.
