Job summary

Civita Health is seeking a Licensed Practical Nurse (LPN) to join the Civita Care Center at Newington, CT. You will deliver high-quality patient care under the supervision of licensed nurses and physicians.

Responsibilities include assessing patient needs, administering medications, wound care, vital signs, and maintaining medical equipment. You'll work with an interdisciplinary team to develop individualized care plans and participate in quality improvement initiatives.

Qualifications

• LPN license or certification for Connecticut.
• Ability to provide high-quality patient care under supervision.
• Experience in sub-acute or long-term care is a plus.

As a Licensed Practical Nurse (LPN) with our organization, you will have the opportunity to deliver exceptional care to our residents while collaborating with a multidisciplinary team of healthcare professionals.

About the Role:

As an LPN at Civita Care Center at Newington, you will be responsible for providing high-quality patient care to our residents, under the supervision of our licensed nurses and physicians. You will be expected to:

• Assess patient needs and develop personalized care plans that address their unique requirements

• Administer medications and treatments as prescribed by physicians

• Provide wound care and maintenance of complex medical equipment

• Conduct vital sign checks and report any changes or concerns to the nursing staff

• Participate in quality improvement initiatives to ensure the highest standards of patient care

Responsibilities:

• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Provide direct patient care, including but not limited to:
  • Vital sign checks
  • Medication administration
  • Wound care
  • Medical equipment maintenance
• Maintain accurate and detailed records of patient care and progress
• Participate in quality improvement initiatives and contribute to the development of policies and procedures
• Demonstrate excellent communication and interpersonal skills when interacting with patients, families, and healthcare staff
