Job summary

Berkshire Center, a skilled nursing facility in West Babylon, NY, is seeking an experienced LPN for a union position with a 3pm–7pm schedule, five days per week and rotating weekends. The role involves direct patient care under supervision, medication administration, and charting, with emphasis on compassionate, detail-oriented nursing for residents.

The ideal candidate will have NYS licensure, LTC experience, and strong teamwork skills to coordinate care within the unit.

Qualifications

• Must be a Licensed Practical Nurse with NYS license.
• Nursing home/LTC experience required; familiarity with geriatric care.
• Ability to deliver compassionate care and communicate effectively with residents and families.
• Strong observation skills and attention to detail to identify condition changes.
• Ability to coordinate unit operations through effective team nursing.

Responsibilities

• Perform clinical tasks under RN/supervisor; monitor vital signs and patient condition.
• Administer injections, prescribed medications, and wound care.
• Record measurements and maintain accurate patient charts.
• Observe residents for progress and medication effects; report changes.
• Provide emotional support and communicate with residents’ families.
