Berkshire Center, a skilled nursing facility in West Babylon, NY, is seeking an experienced LPN for a union position with a 3pm–7pm schedule, five days per week and rotating weekends. The role involves direct patient care under supervision, medication administration, and charting, with emphasis on compassionate, detail-oriented nursing for residents.
The ideal candidate will have NYS licensure, LTC experience, and strong teamwork skills to coordinate care within the unit.
SCHEDULE: 3pm-7pm 5 days per week with rotating weekends.
Berkshire Center is a skilled nursing facility located in West Babylon, NY. Our mission is to provide the highest level of care for each of our residents. We are looking for an experienced LPN that can provide a high level of care. This is a union position.
The purpose of your job as a Licensed Practical Nurse is to perform a variety of care practices under the direction of the Registered Nurse and/or Supervisor of the unit/shift. This includes, but not limited to:
Take and record measurements of blood pressure, temperature, heart rate etc.
Observe residents under treatment to identify progress, side-effects of medications etc.
Monitor residents’ condition including fluid intake and output and compose patient charts.
Administer injections, prescribed medications, enemas etc., clean and dress wounds.
Collect samples for lab testing as assigned.
Provide emotional and psychological support when needed.
Communicate with residents’ family or friends to provide advice, comfort, and release instructions.

Nursing home or LTC experience required
Currently licensed by the State of New York..
Possesses attention to detail and observation skills that enable the recognition of condition changes in the resident.
Ability to coordinate all functions in the operation of a resident unit through effective team nursing assignments.
