Job summary

Discovery Commons Orland Park is seeking a Licensed Practical Nurse (LPN) for the PM 2nd shift. You will provide hands-on nursing care to residents, administer medications, assess health status, and coordinate with the care team to ensure high-quality, person-centered services in a warm senior living community.

The role requires an active LPN license, at least two years of nursing experience, and the ability to work nights and weekends.

Qualifications

• Current Licensed Practical Nurse (LPN) license.
• Minimum of two (2) years of licensed nurse experience.
• Experience in assisted living, long-term care, or skilled nursing preferred.
• Strong clinical judgment, problem-solving, and decision-making skills.
• Ability to work nights, weekends, and flexible schedules as required by the community.
• Proficiency with electronic documentation systems and basic computer applications.

Responsibilities

• Provide nursing care to residents per established care plans.
• Conduct ongoing assessments and monitor changes in condition.
• Administer medications and treatments as prescribed and document accordingly.
• Coordinate care with physicians, families, pharmacies, and other providers.
• Support health promotion, disease prevention, and wellness initiatives.
