Qualifications

• Must have a current, unencumbered LPN license in Ohio.
• Preferred nursing experience in hospital, acute skilled nursing facility, or related medical setting.
• Knowledge of long‑term care laws, regulations, and guidelines.

Responsibilities

• Promote residents’ well‑being and satisfaction by supporting resident‑centered care and the plan of care.
• Communicate changes in resident care plans and participate in care conferences.
• Administer medications and perform treatments per policies and standards of care.
• Document assessments and progress notes in the EMR with dates and signatures.
• Assist with activity planning and coordination to support resident life in the household.
• Uphold Person‑Centered Care philosophy and collaborate with the interdisciplinary team.
