Job summary

Masonicare at Mystic in Mystic, CT seeks an LPN for the Evening Shift (24 hrs/week, end of week). The role includes contributing to care plans, communicating with residents and families, and supervising medication management under supervision.

A sign-on bonus of 1,500 is offered and benefits are provided. Minimum qualifications include graduation from an accredited nursing program, 1–2 years clinical experience preferred, and an active LPN license with CPR certification.

Qualifications

• Graduate of an accredited School of Nursing.
• 1-2 years clinical experience preferred.
• Current LPN license; CPR certification required.

Responsibilities

• Contributes to care plans by collecting data and attending care planning meetings.
• Communicates with residents, families and team to meet resident needs.
• Provides education and counseling to residents and families regarding care needs.
• Documents information in client service records timely and concise.
• Oversees medication management under supervision of RN/designee.
• Performs other duties as assigned.
