Great Lakes Management Co. is seeking a Licensed Practical Nurse (LPN) for a full-time PM shift in Minnetonka, MN. The role focuses on caring for seniors in assisted living, ensuring quality life, safety, and regulatory compliance.

Qualifications

• Current MN LPN license is required.
• Minimum of one year's experience in home health, assisted living or long-term care preferred.
• Good organizational skills with the ability to work with minimal supervision.
• Caring attitude with a passion to care for the elderly.

Responsibilities

• Assures residents meet admission criteria and monitors needs, recommending placements if necessary.
• Ensures residents have up-to-date care plans per state regulations.
• Monitors and maintains resident charts and clinical progress notes.
• Communicates changes in resident needs to physicians, staff and family.
• Facilitates coordination of resident services with other providers.
• Oversees medications and renewals and follows up with physicians.
• Ensures medication administration procedures are followed correctly.
• Documents information in the resident log for staff awareness.
• Provides alternating supervisory visits in coordination with the RN.
• Works as a team with the residents’ care team and supervises medication practices.
• Ensures staff understand and follow all care procedures.
• Shares resident and family concerns with appropriate staff.
• Works with management to assure emergency procedures are in place.
