Qualifications

• Current state LPN license required (MN).

Responsibilities

• Ensure residents meet admission criteria and monitor accordingly.
• Maintain up-to-date care plans per state regulations.
• Monitor resident charts and progress notes.
• Communicate changes to physicians, staff and family.
• Coordinate resident services with other providers.
• Oversee medications and renewals; follow up with physicians.
• Ensure proper medication administration procedures are followed.
