Job summary

CFG Health is seeking a Licensed Practical Nurse (LPN) to provide direct care under supervision in a facility setting. You will support patient care activities, administer medications, monitor conditions, and assist with treatment plans while following clinical protocols.

Responsibilities include documenting in EHR/EMR, communicating changes to the care team, and upholding HIPAA and safety standards. Previous nursing experience in varied care settings is a plus.

Qualifications

• Graduate of an accredited Practical Nursing Program.
• Current, active Licensed Practical Nurse (LPN) license in the state of practice.
• Current CPR/BLS certification.
• Knowledge of nursing principles, practices, and procedures.
• Ability to follow clinical protocols, physician orders, and regulatory standards.
• Strong communication, interpersonal, and teamwork skills.

Responsibilities

• Deliver basic nursing care to patients in accordance with established policies and procedures.
• Monitor and record vital signs, symptoms, and responses to treatment.
• Administer medications, injections, and treatments as ordered within scope of practice.
• Assist RNs or physicians with patient assessments and care planning.
• Collect specimens and perform basic diagnostic tests.
• Provide wound care, dressing changes, and implement infection control measures.
• Maintain accurate, timely documentation in electronic health records (EHR/EMR).
• Educate patients on medications, treatments, and self-care strategies.
• Observe and promptly report changes in patient condition to supervising staff.
• Maintain a safe, clean, and therapeutic care environment.
• Collaborate effectively with interdisciplinary healthcare team members.
• Adhere to HIPAA, OSHA, and all applicable state and federal regulations.
