Job summary

Qualifications

• 1-2 years hospital social work experience preferred.
• Master’s degree in Social Work (MSW) required.
• Current valid driver’s license required.
• Behavioral Health Unit: CDAC (Certified Drug & Alcohol Counselor) certification preferred.

Responsibilities

• Communicates accurate information with patients, families and co-workers at handoffs and other times as needed.
• Assesses patient/family social, emotional and economic factors during admission and ongoing evaluations.
• Assists patients/families in accessing resources and removing barriers to care.
• Participates in multidisciplinary team meetings to advocate for patients and educate teams on psychosocial issues.
• Documents in the medical record accurately and timely according to standards.
• Maintains knowledge of community resources for care planning.
• Provides counseling and supports patients/families dealing with illness, grief, or mental health concerns.
