Job summary

MercyOne West Des Moines Medical Center is seeking a dedicated Social Worker to deliver psychosocial care across the continuum, from admission to post-discharge follow-up. You will assess patient needs, develop plans with the care team, and guide families through available resources while maintaining thorough documentation.

Responsibilities include crisis intervention, counseling, and interdepartmental collaboration to ensure optimal social and health outcomes for diverse patients and families

Qualifications

• Bachelor’s degree in Social Work (required).
• Understanding of medical terminology.
• Proficient in clinical interviewing techniques.
• Familiarity with social work principles, standards, and ethics.
• Basic understanding of Medicare/Medicaid regulations and related laws (desirable).
• Proof of mandatory reporter abuse training within 3 months of hire.

Responsibilities

• Obtain social history information and identify patient needs through interviews and records.
• Analyze patient’s psychosocial situation and needs.
• Develop individualized psychosocial case plans with the healthcare team.
• Coordinate discharge planning and follow-up care.
• Provide information on resources and assist with referrals.
• Provide crisis intervention and supportive counseling as needed.
• Document all social work activity in patient records.
• Collaborate with departments to support patient care.
