Job summary

Qualifications

Responsibilities

• Conduct psychosocial assessments addressing emotional, social, financial, and environmental needs.
• Provide counseling, crisis intervention, and coping support for patients and families.
• Identify discharge barriers related to behavioral health, housing, safety, or guardianship.
• Coordinate community resources and transportation, housing, and financial assistance.
• Collaborate with Care Managers and interdisciplinary teams to develop safe transition plans for high-risk patients.
