Job summary

Hackensack Meridian Health At Home is seeking a dedicated registered nurse or licensed practical nurse to provide high-quality in-home care. The role focuses on one patient at a time, offering a flexible schedule with meaningful patient interactions in home settings.

Position requires NJ RN license or LPN experience, basic computer skills, and strong communication abilities. The role emphasizes safety, documentation, and collaboration with the care team.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• Minimum of six months RN experience or one year of LPN experience, preferably in home care or hospice.
• Excellent written and verbal communication skills.
• Proficient computer skills including Microsoft Office and Google Suite.

Responsibilities

• Provide professional nursing care to patients in their home after normal business hours or weekends.
• Follow physician orders; ensure visit frequency and accurate documentation.
• Assess, plan, implement and evaluate patient care per policy and standards.
• Coordinate with interdisciplinary team for care plans and resource referrals.
• Document care plans, assessments, and plan of care updates timely.
• Supervise home health aide staff and participate in evaluations.
• Maintain updated knowledge of standards and organizational competencies.
• Lift up to 50 lbs., push/pull 50 lbs., stand 8 hours daily.
