Job summary

Hackensack Meridian Health At Home is seeking a dedicated registered nurse to provide professional nursing care in patients' homes, delivering a high-quality continuum of care with a focus on patient-centered outcomes.

You will conduct assessments, develop and implement plans of care, and work with an interdisciplinary team to ensure delivery of safe, effective care in the patient’s home environment.

Qualifications

• Associate's Degree in Nursing or Nursing Diploma.
• Minimum of six months RN experience or at least 1 year of LPN experience, preferably in home care or hospice.
• PC experience/skills.

Responsibilities

• Provide professional nursing care in patients' homes with ongoing assessment and Plan of Care.
• Ensure compliance with physician orders and documentation.
• Document care plans, interventions, and outcomes accurately.
• Coordinate with physicians and interdisciplinary team for patient needs.
• Maintain appropriate visit frequencies and secure authorizations as needed.
• Supervise and support home health aides per regulatory timeframes.
• Adhere to organizational competencies and safety standards.
