Job summary

Hackensack Meridian Health At Home is seeking a registered nurse to provide professional home nursing care after normal business hours and weekends. You will evaluate patient needs, develop plans of care, and coordinate with physicians and an interdisciplinary team to ensure high-quality, safe in-home care.

The role requires an NJ RN license and at least 6 months of RN or 1 year of LPN experience, preferably in home care or hospice, along with strong communication and computer skills.

Qualifications

• Requires RN licensure in NJ and ability to perform home health care.
• Minimum 6 months RN or 1 year LPN experience, preferably in home care or hospice.
• Excellent written and verbal communication skills.
• Proficient computer skills including MS Office and Google Suite.

Responsibilities

• Provide professional nursing care to patients in their home after hours or weekends.
• Ensure adherence to physician orders and follow-up with documentation.
• Document care plans and progress in medical records and care management systems.
• Collaborate with interdisciplinary team to coordinate services and referrals.
• Supervise and provide guidance to home health aides as needed.
• Maintain awareness of updated standards and incorporate into practice.
