Job summary

Hackensack Meridian Health Home Care is seeking a dedicated nurse to provide professional care in patients' homes across New Jersey. You will assess, plan, implement, and evaluate individualized home health plans with an emphasis on high-quality, patient-centered care.

The role includes collaboration with a multidisciplinary team, adhering to policies, and ensuring safe, effective care with timely documentation. Relevant RN/LPN experience and a NJ license are required.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• Minimum of six (6) months RN experience or at least 1 year of LPN experience, preferably in home care or hospice.
• PC Experience/Skills.

Responsibilities

• Provide professional nursing care to patients in their home.
• Assess, develop, implement and evaluate home nursing care needs.
• Coordinate with interdisciplinary team and physicians to Plan of Care.
