Job summary

Concierge Home Care is seeking an experienced Registered Nurse (RN) to provide skilled evening home health visits throughout Clay County. This role is ideal for nurses who enjoy working independently while helping patients remain safely at home.

Schedule Monday-Friday 4:00 PM-10:00 PM with no overnight shifts. Earn $48-$50 per visit plus a $10 evening differential, data stipend, and AI-assisted charting to reduce documentation. Orientation and mentorship provided.

Qualifications

• Active Florida RN license or Compact RN license.
• Valid driver’s license and reliable transportation.

Responsibilities

• Complete SOC admissions and skilled nursing revisits.
• Residency ROC visits and comprehensive patient assessments.
• Develop and update individualized care plans and wound care.
• Manage IV starts and disease management education.
• Coordinate care with physicians and interdisciplinary teams.
• Complete OASIS documentation and EMR charting.
• Participate in rotating evening/weekend on-call coverage.
