Allina Health is seeking an experienced Home Care RN to join a supportive team serving the Hutchinson area. This evening home care/hospice role offers independence, meaningful patient connections, and a schedule designed to keep weekends free.

The position covers an evening/night shift with on-call elements and a 0.75 FTE (60 hours per two-week pay period). Nurses provide comprehensive assessments and coordinated care in community settings.

Competenze

• Associate's or Vocational degree in Nursing
• 0 to 2 years RN experience
• Bachelor's degree in Nursing (preferred)
• 0 to 2 years homecare/hospice experience (preferred)

Mansioni

• Comprehensive assessment of patient and family needs; establish plan of care and modify as needed.
• Maintain education and competencies to provide high quality patient care.
• Incorporates evidence-based nursing practice considering current statutes and regulations.
• Collaborates with family and patient to set realistic, measurable outcomes and continuity of care.
• Coordinates, delegates, and ensures safe, effective care; fulfills organizational requirements.
