Allina Health is seeking a Home Care RN to join the Shakopee Team. The role provides an 8-hour day shift with 1.0 FTE (80 hours per two-week pay period) and rotation every 5th weekend, in a community home care setting with Minneapolis address on file.

The RN will conduct comprehensive patient assessments, establish and adjust care plans, and collaborate with families to achieve outcomes. LPN? No; RN licensure required with BLS; CHPN and specialty certs preferred.

Formación

• Associate's or Vocational degree in Nursing.
• 0 to 2 years RN experience required.
• MN RN license required at hire.
• BLS certification (AHA/ARC or in-house) required.

Responsabilidades

• Comprehensive assessment of patient and family needs and outcomes; develop and monitor the plan of care.
• Coordinate, delegate, and supervise care to ensure safe, effective delivery.
• Maintain competencies; apply evidence-based nursing practice per statutes and regs.
• Collaborate with family/patient to set realistic outcomes and continuity of care.
• Additional duties: charge nurse or preceptor roles as needed.
