Expected to document visits, report changes to supervisors, and comply with agency, state and federal education requirements. 1.0 FTE with 40 hrs/week and occasional weekend shifts.

Qualifications

• Must be 18 years or older with required competencies.
• 0 to 2 years as Nursing Assistant or Home Health Aide.

Responsibilities

• Provides routine, individualized personal care to Home Health, Hospice and Residential Hospice patients.
• Monitors and reports changes in patient health status to Case Manager.
• Assists with basic residential household activities.
