Job summary

Hackensack Meridian Health is seeking a Deficiency Tracking/Susp Analyst for an onsite evening shift in New Jersey. You will compile, organize, and analyze patient medical records to support accuracy, timeliness, and regulatory compliance across CMS, NJDOH, TJC, and HMH policies.

Primary duties include chart retrieval, scanning, QC, and information verification. The role requires strong organizational and communication skills, proficiency with Epic, and the ability to handle sensitive patient

Qualifications

• High School diploma or GED required.
• Good organizational and communication skills.
• Computer literate with MS Office and Google Suite.
• Excellent written and verbal communication.
• Able to bend, lift up to 10 lbs and travel within the HMH network.

Responsibilities

• Retrieves paper records from nursing units for discharged patients.
• Uses Epic chart reconciliation to ensure charts are accounted for.
• Prepares charts for scanning and labeling documents.
• Scans documents into the medical record with data integrity.
• Performs QC and reindexes scanned images when needed.
• Investigates and corrects patient information across hospital applications.
• Assists with medical record merges as per policy.
• Handles release of information requests and MyChart support.
• Answers department phones and routes requests appropriately.
