Job summary

Hope Haven, Inc. is seeking Direct Support Professionals (DSP) to assist clients in meeting personal goals and daily living needs. Shifts are 5:30pm–8:00pm, three days per week, offering an entry point into human services and meaningful community involvement.

You'll provide support per individual service plans, help with daily activities, documentation, transportation as needed, and promote safety and teamwork within the agency and community.

Qualifications

• High school graduate or GED/HSED preferred.
• Good written and verbal communication skills.
• Basic computer skills required.
• Valid driver’s license required (Class C, or Class D if vehicle capacity ≥8).
• Reliable transportation and ability to pass health assessments and background checks.

Responsibilities

• Provide support and training per the service plan and meet physical and mental needs (hygiene, bathing, toileting, escort, transitions, meals).
• Teach self-control and self-management abilities per training.
• Complete client logs and charts within shift or within 24 hours.
• Support development of daily activity schedules and assist with appointments and transportation.
