Job summary

Lakeview Specialty Hospital & Rehab in Westfield, WI (Millpond) is seeking a Community Integration Specialist for the 3pm–11pm shift. CNA and RA certifications are required or preferred.

You will join an interdisciplinary team to provide education, supervision, safety, and behavior management for program participants, support rehabilitation activities, and document progress in ISP records.

Qualifications

• Knowledge of brain injury sequelae and other neurological disorders.
• CNA and RA certifications preferred.
• CBRF certifications may include Medication Administration, Fire Safety, First Aid/Choking and Standard Precautions.

Responsibilities

• Provide education, assistance, supervision, safety, and behavior management for program participants.
• Tend to participants' immediate safety needs and reduce health care errors.
• Document changes and behavior in ISP and related forms.
• Participate as part of an interdisciplinary treatment team.
• Schedule and provide clinical services to meet resident needs.
