Job summary

Escondido Post Acute is seeking a dedicated Certified Nursing Assistant to provide routine nursing care for residents in a full-time PM shift. You will identify residents, follow care plans, and assist with daily living activities while maintaining a safe and compassionate environment.

The role emphasizes strict adherence to privacy, incident reporting, and collaboration with nursing staff to ensure high-quality care for residents in a post-acute setting. CPR certification is preferred.

Qualifications

• Must be a licensed Certified Nursing Assistant in accordance with state laws.
• CPR certification preferred.

Responsibilities

• Provide routine daily nursing care and services per resident assessments and care plans.
• Identify residents with wristband/photo card; document accurately using approved abbreviations.
• Report changes in residents' condition to Nurse Supervisor/Charge Nurse.
• Maintain confidentiality of resident information and report any disclosures.
• Assist residents with daily living activities and mobility under supervision.
• Prepare residents' rooms and assist with transfers, admissions, and discharges.
• Observe for pressure ulcers and report injuries or changes.
• Follow policies and procedures and cooperate with inter-departmental teams.
