Job summary

Great Plains Post Acute seeks a dedicated Certified Nursing Assistant to provide routine daily nursing care and services to residents according to individualized care plans. You will identify residents, document changes, and support a calm, respectful environment on your shift.

Responsibilities include assisting with daily activities, reporting changes to the Nurse Supervisor, maintaining confidentiality, and helping prepare residents for tests and transport as needed.

Qualifications

• Must possess minimum 10th grade education.
• Ability to write reports and procedure manuals.

Responsibilities

• Provide routine daily nursing care per resident care plans.
• Identify residents before procedures or treatments using wristband/photo ID.
• Report changes in condition to Nurse Supervisor/Charge Nurse promptly.
• Maintain confidentiality of resident information and report disclosures.
• Assist residents with daily activities and personal care.
• Transport residents to rooms, tests, or to the receiving area.
• Observe for skin breakdown and report to prevent ulcers.
• Assist with preparing residents for medical tests and examinations.
• Record data accurately on flow sheets and charts.
