Responsibilities

• Assists residents with activities of daily living and daily care tasks.
• Records vital signs and weights per policy.
• Assists with ambulation, transfers and positioning.
• Uses adaptive equipment as directed.
• Provides meals and hydration and notes intake.
• Transports patients to appointments and rehab areas.
• Notifies nurse of changes in patient condition.
• Participates in care planning and follows care plans.
• Performs basic skincare and dressing applications as allowed.
• Maintains patient confidentiality and PHI security.
