Job summary

Responsibilities

• Provide assessments, mental health evaluations, and therapy as part of program goals.
• Complete accurate and timely documentation and meet billable requirements.
• Coordinate with field-based services and transportation as needed.
• Respond to after-hours calls for ACT clients and collateral contacts.
• Conduct crisis-focused assessments and risk evaluations, with stabilization planning.
• Determine and coordinate appropriate levels of care during crises in collaboration with partners.
