Qualifications

• Pharmacist license required; PA licensure per state law.
• At least 1 year of pharmacist experience or a PGY-1 residency.
• Strong teamwork and ability to work under stress.

Responsibilities

• Ensure quality of care through formulary and medication management projects.
• Evaluates patient profiles for appropriateness of therapy and documents drug therapy recommendations.
• Monitors and guides pharmaceutical care through staff development and education.
• Supports patient care functions across the network, especially in specialty areas.
• Promotes ongoing clinical knowledge and staff education.
