Job summary

Lehigh Valley Health Network in Pennsylvania is seeking a Clinical Pharmacist for per diem evening shifts in the Pharmacy Department. You will support formulary management, medication use, and regulatory initiatives, while collaborating with the Therapeutics Committee and clinical staff.

The ideal candidate holds a PharmD or Bachelor of Pharmacy, at least 1 year of pharmacist experience or PGY-1 residency, and PA licensure.

Qualifications

PharmD or BS in Pharmacy.
1 year pharmacist experience or 1 year PGY-1 residency.
Ability to dispense and monitor medications.
Knowledge of laws and regulations related to controlled substances and pharmacy security.

Responsibilities

Ensures quality of care through formulary and/or medication management and quality improvement projects.
Evaluates patient medical profiles for appropriateness of medication therapy based on history, age, and status, and documents drug therapy recommendations.
Monitors, evaluates, and guides the provision of pharmaceutical care through staff/resident/student development and education.
Supports patient care functions of the network, especially in area of specialty.
Demonstrates ongoing clinical knowledge, professional development, and promotion of the pharmacy profession through training staff and/or students and residents.
