Job summary

TPC Wellness in Ironton, OH is seeking a part-time Case Manager to support clients with resources, recovery goals, and daily living tasks.

Under supervision of the Lead Case Manager, you will manage caseloads, perform community and office-based support, and ensure compliance with policies. The role requires CDCA certification or ability to obtain within 30 days, valid driver’s license, and strong organizational and communication skills.

Qualifications

• Valid driver's license.
• High school diploma or equivalent.
• Active CDCA certification or ability to obtain within 30 days of hire.

Responsibilities

• Manage assigned caseloads and provide client contact.
• Monitor progress and assess ongoing needs per policies.
• Meet with clients individually and in groups.
• Collaborate with families, clinicians, and community partners.
• Provide crisis intervention, advocacy, and resource coordination.
• Conduct community visits and transport as needed.
• Develop service plans and document using EHR systems.
• Maintain accurate, timely client records.
• Participate in Loved Ones Group and family outreach.
• Communicate progress with probation and referral sources.
• Attend trainings, staff meetings, and agency events.
• Contribute to a safe, recovery-focused environment.
