Job summary

TPC Wellness in Mount Orab, OH is seeking a compassionate Case Manager to join our recovery-focused team. You will manage caseloads, provide client and family support, and ensure documentation in the EHR system is complete and compliant.

Qualifications

• Valid driver’s license required.
• Active CDCA certification or ability to obtain within 30 days of hire.

Responsibilities

• Maintain assigned caseloads and ensure regular client contact.
• Monitor client progress and assess ongoing needs in accordance with agency policies.
• Meet with clients individually and in group settings.
• Collaborate with families, clinicians, case managers, and other community partners.
• Provide crisis intervention, advocacy, resource coordination, and ADL assistance.
• Conduct community visits, client monitoring, and transportation as needed.
• Develop service plans and complete clinical documentation using EHR systems.
• Maintain accurate, timely, and compliant client records.
• Conduct family outreach and participate in the Loved Ones Group.
• Communicate client progress with probation departments and referral sources as appropriate.
• Attend trainings, staff meetings, and agency events.
• Contribute to a safe, recovery-focused environment.
