Gilead Community Services Inc. is seeking a caring Clinical Case Manager for its Old Saybrook adolescent residential program.

The role involves case management, advocacy, and supportive services, with a schedule of Sunday & Monday 3:00 PM - 11:00 PM and Friday & Saturday 3:00 PM - 11:00 PM, totaling 32 hours per week. The position requires a bachelor’s degree in a human service field, at least one year of clinical experience, and a valid driver’s license.

Qualifications

• Bachelor’s degree in a human service field.required
• Master’s degree preferred for advancement and supervision responsibilities
• One year of clinical experience is preferred in the role

Responsibilities

• Understand needs of clients with severe mental health and substance use disorders through training and education.
• Assist with daily living tasks: meal prep, housekeeping, shopping, cleaning, laundry, and money management.
• Provide recovery-oriented services emphasizing dignity, respect, and inclusion of family and natural supports.
• Coordinate and provide transportation to treatment, community, and other appointments.
• Collaborate with the treatment team to support clients’ mental health and assess risks.
• Act with compassion, integrity, and ethics.

Education

Bachelor’s degree in a human service field
