Aspire Talent Solutions is seeking an Admissions Nurse (RN) in Connecticut for an on-site, full-time role with an evening shift from 3:00 PM to 11:00 PM. This position supports a mission-driven senior living and healthcare organization.

The RN will oversee clinical intake, initial assessments, medication reconciliation, and electronic chart setup to ensure safe transitions for new residents while coordinating with discharge planners, physicians, leadership, and families.

Qualifications

• Active, unrestricted RN license in Connecticut.
• 1–2 years nursing experience in SNF/LTC/sub-acute rehab preferred.
• Prior admissions, discharge coordination, or desk nursing experience is desirable.
• Hands-on experience with EMR systems (e.g., PointClickCare) for rapid chart initiation.
• Strong organizational, prioritization, and interpersonal skills to build trust with families and hospital staff.

Responsibilities

• Lead clinical intake and onboarding for evening admissions (3:00 PM – 11:00 PM).
• Conduct initial nursing assessments, skin evaluations, and admission documentation.
• Reconcile discharge summaries, orders, and pharmacy profiles for medication continuity.

An established, mission-driven senior living and healthcare organization in Connecticut is seeking a dedicated, detail-oriented Admissions Nurse (RN) to oversee and streamline clinical intake for incoming residents.

Specializing in evening admissions (3:00 PM – 11:00 PM), this role coordinates directly with hospital discharge planners, attending physicians, facility leadership, and families to ensure safe, seamless transitions into the center. By focusing specifically on incoming clinical assessments, medication reconciliation, and initial electronic chart setups, you enable floor staff to focus on direct resident care while setting a high standard of clinical excellence from day one.

Key Responsibilities

• Lead clinical intake and onboarding procedures for all incoming admissions during the evening shift (3:00 PM – 11:00 PM)
• Conduct initial nursing assessments, skin evaluations, and comprehensive admission documentation upon resident arrival
• Reconcile hospital discharge summaries, attending physician orders, and pharmacy profiles to ensure immediate medication continuity
• Partner closely with hospital discharge planners, interdisciplinary care teams, and resident families to prepare room setups and specialized clinical equipment
• Initiate preliminary individualized care plans and communicate critical resident needs to on-duty nursing supervisors
• Orient new residents and families to facility policies, care team structures, and clinical routines to ease transition anxiety

Qualifications

• Licensure: Active, unrestricted Registered Nurse (RN) in the State of Connecticut
• Clinical Experience: Minimum of 1 to 2 years of nursing experience within a skilled nursing facility (SNF), long-term care (LTC), or sub-acute rehabilitation environment preferred
• Admissions Capability: Prior experience handling admissions, hospital discharge coordination, or unit management/desk nursing is highly desirable
• EMR Proficiency: Hands-on experience with Electronic Medical Record systems (e.g., PointClickCare) for accurate, rapid chart initiation
• Communication: Exceptional organizational, clinical prioritization, and interpersonal skills to build trust quickly with families and hospital representatives
