Job summary

The Keystone Distinction Group is seeking an Admissions Nurse (RN) in Connecticut for the evening shift (3:00 PM – 11:00 PM). You will oversee clinical intake, initial assessments, medication reconciliation, and chart initiation to ensure a seamless transition for incoming residents.

In this role, you will collaborate with hospital discharge planners, physicians, and families, establishing care plans and coordinating with the care team to set up rooms and equipment.

Qualifications

• Active Registered Nurse license in the State of Connecticut
• Experience with admissions, discharge coordination, or unit desk nursing is desirable
• Proficiency with electronic medical record systems for rapid chart initiation

Responsibilities

• Lead clinical intake and onboarding for evening admissions (3:00 PM – 11:00 PM)
• Conduct initial nursing assessments and admission documentation
• Reconcile discharge summaries and orders for immediate medication continuity
• Coordinate with hospital discharge planners, care teams, and families for room setups and equipment
• Develop preliminary individualized care plans and communicate needs to on-duty supervisors
• Orient new residents and families to policies and routines
