Job summary

Key People in Connecticut seeks an Admissions Nurse (RN) for evening intake and onboarding. The role focuses on admissions, assessments, medication reconciliation, and electronic chart setup to support safe transitions into the facility.

The RN will coordinate with hospital discharge planners, physicians, and families, ensuring smooth room setup and timely care planning. Strong EMR skills and exemplary communication are required.

Qualifications

• Active Registered Nurse license in Connecticut
• 1–2 years nursing experience in SNF/LTC or sub-acute rehab preferred
• Experience with admissions, discharge coordination, or desk nursing is preferred
• Hands-on EMR experience for rapid chart initiation and documentation

Responsibilities

• Lead evening clinical intake and onboarding for admissions (3:00 PM–11:00 PM)
• Perform initial nursing assessments and admission documentation
• Reconcile discharge summaries and physician orders for immediate med continuity
• Coordinate with discharge planners, care teams, and families for room setup and equipment
• Initiate preliminary care plans and communicate resident needs to on-duty supervisors
• Orient new residents and families to policies and routines
