Job summary

Kaweah Health in Visalia, CA seeks an ASW/AMFT for the Emergency Department night shift. The role involves psychosocial assessments, crisis intervention, discharge planning and collaboration with physicians and care teams to support patients and families.

Required: California Board of Behavioral Sciences registration as ASW or AMFT, crisis intervention training within 60 days, licensure within six years. This is a full-time, night-shift position under the Patient & Family Services department.

Qualifications

• Registration with California Board of Behavioral Sciences as ASW or AMFT.
• Crisis intervention training within 60 days of hire/transfer.
• Licensure within six years of hire/transfer.

Responsibilities

• Intervenes in crisis situations, providing needed assessment, counseling and information to patients and their families.
• Gathers information about the patient's social, psychological, cultural, environmental and financial situation for a psychosocial assessment and treatment plan.
• Counsels individuals and their families regarding psychosocial needs and problems related to their medical condition.
• Makes rounds and responds to referrals from physicians and staff providing guidance to necessary social services.
• Provides consultation to physicians, hospital staff and community as it relates to patient care needs.
• Attends team conferences and rounds and coordinates discharge planning and linkage to appropriate resources.
