• Provides discharge planning, coordinating home care, transportation and follow-up.
• Documents activity in medical record according to policy.
• Intervenes in crisis situations, providing assessment, counseling and information to patients and families.
• Provides consultation to physicians, hospital staff and community on patient care needs.
