Job summary

Kaweah Health in Visalia, CA is seeking a licensed ASW/AMFT to provide psychosocial assessments and crisis intervention across our hospital and clinic system. The ASW/AMFT will conduct assessments, counseling, and discharge planning, coordinating with physicians and social work staff to support patients and families.

The role requires registration with the California Board of Behavioral Sciences and licensure within six years of hire, with ongoing crisis intervention training provided by Kaweah

Qualifications

• License /Certification Required: Registration with California Board of Behavioral Sciences as ASW or AMFT.
• Licensure within six years of hire/transfer. Kaweah Health approved crisis intervention training within 60 days of hire/transfer.

Responsibilities

• Intervenes in crisis situations, providing needed assessment, counseling and information to patients and their families.
• Gathers information about the patient's social, psychological, cultural, environmental and financial situation for a psychosocial assessment and treatment plan.
• Counsels individuals and their families regarding psychosocial needs and problems related to their medical condition.
• Makes rounds and responds to referrals from physicians and staff providing guidance to necessary social services.
• Coordinates discharge planning and links patients to appropriate community resources and services.
• Provides education to department and hospital staff on psychosocial aspects of illness.
