Job summary

Responsibilities

• Conduct comprehensive clinical assessments of patients.
• Develop and implement individualized care plans.

• Educate patients and families about diagnoses and care plans.
• Document patient information in medical records.
• Administer medications and treatments safely.
• Discharge planning coordination.
• Ensure compliance with policies and regulations.
• Address staffing, resource issues and improve processes.
