Qualifications
- Requires RN license and state nursing license.
- Associate's Degree in Nursing is required; BSN preferred.
- English fluency in speech and writing is required.
Responsabilités
- Conduct comprehensive clinical assessments to evaluate patients' health.
- Develop and implement individualized care plans based on assessments.
- Coordinate multidisciplinary care with physicians, nurses, social workers, and therapists.
- Monitor patient progress and adjust care plans as needed.
- Document all patient information accurately in the medical record.
- Educate patients and families about diagnoses and care plans.
- Supervise nursing and support staff and ensure quality of care.
- Coordinate discharge planning for smooth transitions home or to another facility.
