Job summary

Qualifications

Responsibilities
- Develop and implement service programs by assessing patient needs and outlining interventions.
- Conduct thorough patient assessments including history and exams.
- Create individualized care plans with multidisciplinary teams.
- Administer medications and treatments safely as prescribed.
- Monitor outcomes and adjust care plans based on data.
- Educate nursing staff on new protocols and best practices.
- Ensure communication with patients and families about care plans.
- Maintain documentation in electronic medical records.
