Responsibilities

• Assess patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data.
• Develop and implement patient care plans with the interdisciplinary team, observing outcomes.
• Demonstrate competency in IVs, wound care, medications, catheterization, and specimen collection.
• Advocate for patient safety and rights, ensuring high-quality care.
• Provide patient and family education and assess learning needs.
• Engage patients in care through motivational interviewing and teach-back methods.
• Deliver relationship-based care with cultural sensitivity.
• Coordinate group visits for high-risk patients; support transitions and medication reconciliation.
• Contribute to continuous process improvements and patient satisfaction initiatives.
